In accordance with Public Act 099-0278, when you enroll at Sandburg, you’ll have the chance to authorize in writing the disclosure of certain private mental health information to a designated person by completing the student optional disclosure of mental health information authorization form, which grants Sandburg’s designated mental health professional permission to release information about your mental health to the third party listed. You must complete a separate form for each third party you want to grant access to information on your student records. 

The completed form is sent to the Registrar. The authorization to release information doesn’t expire unless you sign the form revoking the authorization. You can revoke the authorization at any time by sending a written request to Sandburg, 2400 Tom L. Wilson Blvd., Galesburg, IL 61401. This form doesn’t allow the third party access to other student record information from the college.

Sandburg may disclose your mental information if a physician, clinical psychologist or qualified examiner employed by the college determines that you pose a clear danger to yourself or others to protect you or another person against a clear, imminent risk of serious physical or mental injury or disease or death being inflicted upon the person or by you to yourself or another person. Within 24 hours of the determination, he physician, clinical psychologist or qualified examiner will attempt to contact the designated person and notify the designated person that the physician, clinical psychologist or qualified examiner determined that you pose a clear, imminent danger to yourself or others.
